HiRes_WOMANPatient: a 29 year old female

Diagnosis: The patient has a diagnosis of hyperlipidemia.

Medical History: The patient has a history of hyperlipidemia with poor diet and exercise compliance for better health.

MCM Intervention: Since enrolling in the DM program the patient has changed her dietary habits by using portion control and eating more fruits and vegetables. Fruits and vegetables contain important nutrients needed by the body. Portion control reduces her daily calorie count. This member also walks for 30 minutes 2 days a week. These changes resulted in a weight loss of sixteen pounds and her BMI dropped 3 points. This reduced the work load placed on her heart. Surveillance and education by the health coach regarding diet combined with exercise enabled this patient to focus on specific goals that resulted in weight loss. This shows the patient is dedicated and committed to preventing the escalation of her condition.
