Diagnosis: The member has a diagnosis of hyperlipidemia.
Medical History: The member has a history of hyperlipidemia.
MCM Intervention: Since enrolling in the DM program the member has received education and surveillance regarding diet, heart healthy cooking methods, and daily exercise. The member was exercising three to four days per week but his weight had reached a plateau. In addition he was using salt to season his foods. Together, this caused him to retain water, prevented him from losing weight, and put strain on his heart. The member met the challenge set by the health coach and increased his exercise to six days per week for sixty minutes. Currently, he walks three to five miles everyday. In addition, he stopped adding salt to his food opting for herbs and pepper. He has added fish and fish oil tablets to increase his good cholesterol. Moreover, the member was able to discontinue his cholesterol medication. Maintaining focus on short term realistic goals has enabled the member to avert a costly medication.
