Diagnosis: The patient has a diagnosis of hypertension, hperlipidemia, and type 2 diabetes.
Medical History: The patient has a history of hypertension, hyperlipidemia, and type 2 diabetes with poor compliance for health.
MCM Intervention: Since enrolling in the DM program the member has received education and surveillance regarding daily aerobic exercise and diet. The patient was not exercising routinely and was eating large portions. This prevented him from losing weight, caused water retention, and added additional strain on his heart. The patient met the challenge set by the health coach and began exercising daily for forty-five minutes. In addition, he began using the plate system which enabled him to limit his portion sizes. His has lowered his blood pressure from 135/85 to 124/74 which is his target. He has lost forty pounds in one year; moreover, he has averted the progression of his condition. Maintaining his focus on short term realistic goals has enabled the member to avert being placed on another costly medication.
