Diagnosis: The patient has a diagnosis of type 2 diabetes, hypertension, hyperlipidemia, and CAD.
Medical History: The patient has a diagnosis of type 2 diabetes, hypertension, hyperlipidemia, and CAD with poor diet and exercise
compliance for better health.
MCM Intervention: Since enrolling in the DM program the patient has changed his dietary habits by controlling portion sizes. Large portions caused weight gain and placed additional strain on his heart. The patient has also been walking for 30 minutes 5 days a week. He has lost a total of 15 pounds and his BMI dropped 2 points. Poor dietary habits can cause an elevated blood glucose level. His A1c level is trending towards his target number. The surveillance and education by the health coach has enabled the patient to focus on correct portion sizes, heart healthy meals, and routine exercise. This shows his dedication to preventing the escalation of his condition.
