Diagnosis: The patient has a diagnosis of hypertension and hyperlipidemia.
Medical History: The patient has a history of hypertension and hyperlipidemia with poor diet and exercise compliance for better health.
MCM Intervention: Since enrolling in the DM program the patient has maintained a heart healthy diet and remained physically active. The patient lifts weights 3 days a week and plays tennis with his 16 year old son. His diet has been rich in seafood, lean meats, vegetables, fruits and nuts. Lean proteins, the right kind of oil found in nuts, and the nutrients found in fruits and vegetables were all part of this patient’s heart healthy diet. When this patient started talking to the health coach he was taking Losartan for his blood pressure. First the MD cut this medication in half. At the last MD visit the medication was discontinued. Surveillance and education provided by the health coach enabled this patient to reach his goal to “get off his blood pressure medication.” This shows the patient’s dedication and commitment to avoiding the cost of his medication and the progression of his condition.
