Success Story: Averted Progression of Disease

HiRes_WOMANPatient: a 64 year old female

Diagnosis: The patient has a diagnosis of hypertension, hyperlipidemia, and diabetes type 2.

Medical History: The patient has a diagnosis of hypertension, hyperlipidemia, and diabetes type 2 with poor compliance for health.

MCM Intervention: Since enrolling in the DM program, this patient has received education and surveillance regarding routine exercise and restricting carbohydrates. Too many carbohydrates make it difficult to control blood sugar and cause weight gain which places additional strain on the patient’s heart. This patient lost 11 pounds, which puts her over the half way mark for reaching her weight loss goal. She has a goal to lose a total of 20 pounds. Her body mass index has dropped from 30 to 28. Her blood pressure went from 142/84 to a healthier range of 120/80. She is walking 1 mile on the even days, and 2 miles on the odd days. She is reaching her goal for keeping her A1c below 7, her last test result was 6.5. These accomplishments show her commitment to averting the escalation of her condition.
