Success Story: Averted progression of disease

HiRes_MANPatient: a 54 year old male

Diagnosis: The patient has a diagnosis of type 2 diabetes and high blood pressure.

Medical History: The patient has a history of type 2 diabetes and high blood pressure with poor compliance for better health.

MCM Intervention: Since enrolling in the DM program, the patient has changed his dietary habits by eliminating fried foods, and reducing his intake of red meats and white rice and bread. These foods are usually high in saturated fat and calories. The surveillance and education by the health coach has enabled the patient to focus on preparing meals in a heart healthy fashion. The patient has also been exercising regularly by walking for 30 – 40 minutes, 4 days per week and has lost 6 pounds since the last health coaching visit. The patient’s A1C has decreased from 9 to 7 and his blood pressure has also decreased to within his target range. This shows the patient’s dedication to preventing the escalation of his condition.
