Success Story: Averted progression of disease

Diagnosis/Medical Condition: Coronary Disease

Medical History: Member entered Health Coaching program with high blood pressure and elevated BMI(weight to height ratio).

MCM Intervention(s): Health Coach provided both written and verbal guidance regarding heart healthy food choices for weight management as well as control of sodium intake due to direct influence on blood pressure. Health Coach encouraged a regular activity plan as Member admitted sedentary lifestyle. Member reported dietary adjustments but “lost control over holidays”. Member also stated drop in activity level due to weather changes. Health Coach encouraged member to work way back to previous adjustments due to noted improvements and made suggestions for various indoor facilities for activity. Member reported to Health Coach that blood pressure was elevated at MD office visit, and doctor attributed to weight gain. Health Coach reviewed initial lifestyle adjustments identified as self goals and made follow up calls to monitor progress and offer encouragement. Member has lost seventeen pounds in 5 months and now belongs to a health club for indoor activities during bad weather. Her blood pressure has dropped and her “doctor is happy with her improvements”.
