Diagnosis: The patient has a diagnosis of high cholesterol. high blood pressure, and diabetes.
Medical History: The patient has a history of high cholesterol, high blood pressure, and diabetes with poor compliance for health.
MCM Intervention: Since enrolling in the DM program, the patient has received education and surveillance regarding meal planning, reading food labels, reducing portion sizes, limiting sweets, and processed and prepared foods. Large portions, sweets, and processed foods are high in calories, saturated fat, and sodium. They make it difficult to regulate blood glucose, cause weight gain, water retention, and place an additional strain on the heart. The patient has decreased his portion sizes, is limiting his starches, and looks for the heart healthy logo on food labels. Additionally, he has maintained his exercise program, working out for 60 minutes daily and his A1c has decreased from 10.1 to 9.4. His fasting blood glucose has dropped 55 points, and his cholesterol values are within normal limits. The surveillance and education by the Health Coach has enabled the patient to focus on portion sized, heart healthy eating in combination with regular exercise. This shows his dedication towards preventing the escalation of his condition.
