Diagnosis/Medical Condition: The patient has a diagnosis of type 2 diabetes, hypertension, and hyperlipidemia.

Medical History: The patient has a history of type 2 diabetes, typertensio, and hyperlipidemia with poor diet, exercise, and provider compliance for health.

MCM Intervention(s): The patient has received education and surveillance regarding compliance with provider follow-up visits as well as maintaining diet and exercise compliance. The patient has reduced portion sizes, is limiting calories to 1800 per day, and is exercising for 30 minutes daily. The patient’s A1C has dropped one full point to her target goal of 7% and her fasting blood glucose has dropped from 145 to 102. The patient has also experienced a 10 pound weight loss in the last several months. The patient’s adherence to routine provider follow-up visits and attention to diet and exercise shows her dedication to preventing the escalation of her condition.
