HiRes_WOMANPatient: a 47 year old female

Diagnosis/Medical Condition: The patient has a diagnosis of coronary disease.

Medical History: Patient entered Health Coaching program with the following cardiac risk factors: elevations in weight and LDL (bad) cholesterol as well as a low HDL (good) cholesterol.

MCM Intervention(s): Health Coach provided education about portion sizes, low fat food choices and cooking methods to reduce fat (cholesterol) intake; low carbohydrate choices to reduce caloric intake; and guidelines for developing a regular exercise plan to increase the HDL levels. Patient readily made lifestyle adjustments. Patient has lost 30 pounds to date. Additionally, she has also started to walk 5 days per week for 30 – 40 minutes. This shows her dedication in averting the progression of her condition. Her MD expects her next lab values will be positively impacted by her efforts.
