Success Story: Averted progression of disease

HiRes_MANPatient: a 42 year old male

Diagnosis: High Cholesterol

Medical History: Patient has been in coaching for less than a year. His cholesterol level was noted to be elevated on a health risk assessment. He takes no medication and only sees the doctor if he is sick. He was not actively involved with exercise.

MCM Intervention: Patient encouraged to move and boost his activity level. Advised to get at least 30 minutes of exercise 5 days a week. Health Coach reviewed guidelines for low fat food selections and cooking methods to reduce fat (cholesterol) intake; low carbohydrate choices to reduce caloric intake; and helped member to develop a regular exercise plan to increase his HDL level. Health Coach provided follow up calls to monitor progress and support efforts in adjusting lifestyle. Educational materials and visual guides were provided via email to help the patient understand serving sizes and portion distortion. He reports he is exercising as of 8 weeks ago. He has lost 5 pounds and enjoys smaller meals. Health Coach encouraged patient to make an appointment to see an MD and see him regularly for care. Patient followed through and will continue to see his physician for checkups annually. Patient has taken steps to improve his health through exercise, change of diet, and timely physician office visits.
