Diagnosis: Coronary Disease and Diabetes
Medical History: The patient had a diagnosis of high blood pressure, high cholesterol, and Type 2 diabetes with poor compliance for health.
MCM Intervention: Health Coach reviewed guidelines for a low sodium (salt) diet to prevent fluid retention which increases blood pressure; low fat food selections and cooking methods to reduce fat (cholesterol) intake; low carbohydrate choices to reduce caloric intake; and guidelines for developing a regular exercise plan to increase the HDL level and burn calories . Patient followed through and made lifestyle adjustments. Health Coach monitored and supported patient during adjustments with follow up calls. Patient has lost 25 pounds. His blood pressure has decreased to 122/72 which is within normal limits. His A1c has also decreased to 6.0 which is within his target range. He stated that he has been receiving a lot of compliments from friends and family. At his recent MD visit, his MD was very pleased and has taken him off all blood pressure, diabetes, and cholesterol medications. The patient’s adherence to a reduced calorie, salt restricted diet and daily exercise program shows his dedication to maintaining a healthy lifestyle and avoiding the escalation of his condition.
