Success Story: Averted progression of disease

HiRes_WOMANPatient: a 64 year old female

Diagnosis: The patient has a diagnosis of hypertension, hyperlipidemia, and diabetes type 2.

Medical History: The patient has a diagnosis of hypertension, hyperlipidemia, and diabetes type 2 with poor compliance for health.

MCM Intervention: Since enrolling in the DM program, the patient has received education and surveillance regarding routine exercise, reducing portion sizes and restricting carbohydrates. Large portions and too many carbohydrates make it difficult to control blood sugar and cause weight gain which places additional strain on the patient’s heart. This patient recently lost 12 pounds and dropped her Body Mass Index by 2 points which brought her closer to a healthy range. Additionally, she is riding a stationary bike for 30-40 minutes most days of the week. The patient’s gradual weight loss is part of her goal to eventually reduce her medications. This shows her commitment to averting the escalation of her condition.
