Success Story: Averted Progression of Disease

HiRes_MANPatient: a 62 year old male

Diagnosis: The patient has a diagnosis of type 2 diabetes and hypertension.

Medical History: The patient has a history of hyperlipidemia with poor compliance for health.

MCM Intervention: Since enrolling in the DM program, the patient has received education and surveillance regarding meal planning for a heart healthy diet and reducing portion sizes. Large portions caused him to gain weight and made it difficult for him to control his blood sugar. He is planning his meals daily to include protein sources low in saturated fat such as fish and skinless poultry and preparing them in a healthy manner. In addition, he is eating fresh vegetables which are high in water content and low in calories. Lastly, he has beenreducing his portion sizes. Over the last two months, his blood sugar has trended down fifty points to a near normal range and his A1C has gone from 9% to 6.2% which is his target. Lastly, the patient has lost eight pounds. The education and encouragement from the health coach has enabled the patient to focus on heart healthy dietary changes. This shows his commitment to a healthy life style and thereby avoiding the progression of his conditions.
