Diagnosis: The patient has a diagnosis of Asthma, Hyperlipidemia, Hypertension and CHF.
Medical History: The patient has a diagnosis of Asthma, Hyperlipidemia, Hypertension and CHF with poor compliance for health.
MCM Intervention: Since enrolling in the DM program the patient has received education and surveillance with regards to her multiple diagnosis. During an assessment with the Health Coach for Asthma the patient reported a chronic dry hacking cough and chronic hoarseness. In addition, the patient had been walking 10,000 steps per day for twenty – two days. When oxygen levels drop, the lungs do not receive an adequate blood supply which causes the blood pressure to rise. Consequently, this increases the workload on the heart and over time, can weaken the heart muscle. The Health Coach advised this patient to make an appointment with the pulmonologist. The patients’ breathing was monitored at night and it was determined she required oxygen during sleep. This patient’s response to the suggestion from the Health Coach to see her pulmonologist improved her health and ability to deal with multiple conditions. It also helped to avert more medical costs, such as doctor visits and hospital admissions.
