Health Risk Assessment
The health risk assessment is based on Healthier People 3.0 of the Carter Center at Emory University. It is approximately forty questions dependant upon the sex and smoking status of the user. A ten page report includes sections on suggested behavioral changes, recommended preventive services, Internet links to risk specific sites, and a doctor chart report that the user can print and take with them to their physician. A member page can be created which allows the user to go back and review previously taken assessments.
General Health Assessment
Again, the Healthier People algorithms are used as the baseline for this assessment and all of the elements of the Health Risk Assessment are factored. Additional elements related to family history, stress and depression have been incorporated as well. This assessment also provides feedback as the assessment is being taken, with a shorter summary document produced at the end. This assessment focuses less on the "numbers" and encourages a move to better habits.
Cardiac Risk Assessment & Diabetes Risk Assessment
Cardiac risk and diabetes risk can be measured with our shorter disease specific assessments. These two assessments are shorter in the number of questions and the results report. Both do a good job of alerting the user to possible higher than average risks for these diseases. Both reports include additional Internet links based on risk and how the user answered specific questions.
General Well-Being (Mental Health Assessment)
The well-being assessment gives users twenty-two questions and reports their overall ratings in stress, depression, general health, and vitality.
Fitness Assessment
The fitness assessment consists of three parts. First the user completes the Physical Activity Readiness Questionnaire (PARQ) to determine whether it is safe for the user to take the more strenuous fitness assessment. After completing the PARQ, an instructional worksheet is provided for the user to perform a three minute step test, muscular endurance (pushups and sit-ups), and a flexibility test. The user then enters their results into the assessment screen and a report is generated.
Aggregate Health Plan Reports
Allows you to see graphically how your population is doing, and where possible shows how you are doing against the Healthier People 2010 goals. This allows us to help you tailor intervention programs specifically for your group, and see what changes will have the biggest impact.
