Disease Management Success Story: Female with Diabetes

Patient: a 61 year old female

Diagnosis: Diabetes & High Blood Pressure

Circumstances: The patient has a history of diabetes and high blood pressure with poor compliance for health.

MCM Intervention: Since enrolling in the DM program, the patient has received education and surveillance regarding routine exercise, heart healthy meal planning, reading food labels, eliminating added salt, reducing portion sizes and restricting carbohydrates. Large portions and too many carbohydrates make it difficult to control blood sugar and cause weight gain, and with added salt, cause water retention and place strain on the heart. The patient is restricting her carbohydrates, reducing portions sizes, reading food labels, and she has eliminated added salt. Additionally, she is exercising 5 days per week for a total of 60 minutes a day alternating walking two times per day during breaks at work for 15 minutes each time, as well as aerobic exercise in the evenings after work.

Outcome: She has lost four pounds since the last HC visit, her cholesterol levels are trending downwards, and her blood pressure remains within her target range. These improvements have inspired her to maintain focus on short term, realistic goals, enabling the patient to avert the progression of her condition.
