Patient: a 59 year old male

Diagnosis: Diabetes

Circumstances: Patient has been treated for this condition for several years. He is overweight and unable to tolerate too much exercise due to painful feet. His blood sugars were as high as 200 and his weight was 292 pounds. He made unhealthy food choices.

MCM Intervention: Patient has been in health coaching for a a year and has made progress with losing weight and reducing blood sugars. Health coach discussed nutrition, portion sizes, and activity as tolerated. He was sent reading material on meal planning.

Outcome: Patient’s weight is now 270 pounds and his blood sugar average is 170. He walks as much as he can daily. He has reduced the risk of heart attack or stroke by changing his lifestyle.
