Disease Management Success Story: 51 y/o with Hypertension and Hyperlipidemia

Patient: a 51 year old male

Diagnosis: The patient has a diagnosis of hypertension and hyperlipidemia.

Circumstances: The patient has a history of hypertension and hyperlipidemia with poor compliance for health.

MCM Intervention: Since enrolling in the DM program, the patient has received education and surveillance regarding routine exercise, reducing portion sizes, reducing processed and prepared foods, and avoiding added salt to foods. Large portions and processed foods are higher in calories, saturated fat, and sodium causing him to gain weight, retain water, and place additional strain on his heart. The patient was not exercising previously, but set a goal to exercise for 20 minutes, 3-4 times a week. The Health Coach also worked with him to schedule needed tests, which he had been putting off for a long time.

Outcome: Patient followed through and made the necessary appointments and lifestyle changes. Maintaining focus on short term, realistic goals has enabled the patient to avert the progression of his condition.
