Disease Management Success Story: 38 y/o Male with High Cholesterol

Patient: a 38 year old male

Diagnosis: High Cholesterol

Circumstances: Patient has had this condition for 4 years. It was first identified from a health history screening provided at the job site.

MCM Intervention: Health coaching began in 2008. A review of heart healthy food choices was discussed. Literature was sent to patient for meal planning, what to look for when reading food labels, and the benefit of physical activity.

Outcome: There has been a gradual reduction of cholesterol numbers toward the target levels and patient did not go on medication. His total cholesterol was reduced from 237 to 219 and the bad cholesterol was down from 156 to 142. He has reduced the risk of heart attack and stroke by changing eating habits and reducing cholesterol.
